They will flee from what they long for most intensely in their fantasies if they encounter it in real life, and they are most likely to abandon themselves to fantasies when they no longer need to fear their realization.

When one recognizes the existence of all this hopelessness it should not be difficult to understand what appears to be an excessive aggressiveness or even meanness, unexplainable by the particular situation. A person so shut out from every possibility of happiness would have to be a veritable angel if he did not feel hatred toward a world he cannot belong to.
If man is the more normal, healthy and happy, ... the more successfully he can repress, displace, deny, rationalize, dramatize himself and deceive others, then it follows that the suffering of the neurotic comes ... from painful truth. ... Spiritually the neurotic has been long since where psychoanalysis wants to bring him without being able to, namely at that point of seeing through the deception of the world of sense, the falsity of reality. He suffers, not from all the pathological mechanisms which are psychically necessary for living ... but in the refusal of these mechanisms, which is just what robs him of the illusions important for living.
The expectation that every neurotic phenomenon can be cured may, I suspect, be derived from the layman's belief that the neuroses are something quite unnecessary which have no right whatever to exist. Whereas in fact they are severe, constitutionally fixed illnesses, which rarely restrict themselves to only a few attacks but persist as a rule over long periods throughout life.
Neurosis was caused by our attempt to separate physical and metaphysical levels, to set them up in opposition to each other, thus engaging in an internecine war. If it is true that we do live on several levels simultaneously- drama and action, past and present, personal and collective- we are given ways to unify them: one by religion, the other by art. Separating such levels is only necessary when they conflict, and separation is a result of conflict. Seeing how these levels can work together in harmony is the task of our contemporary writers.
This tendency to avoid problems and the emotional suffering inherent in them is the primary basis of all human mental illness. Since most of us have this tendency to a greater or lesser degree, most of us are mentally ill to a greater or lesser degree, lacking complete mental health. Some of us will go to quite extraordinary lengths to avoid our problems and the suffering they cause, proceeding far afield from all that is clearly good and sensible in order to try to find an easy way out, building the most elaborate fantasies in which to live, sometimes to the total exclusion of reality. In the succinctly elegant words of Carl Jung, “Neurosis is always a substitute for legitimate suffering.”2

We recoil from the notion that this is it — that this life, with all its flaws and inescapable vulnerabilities, its extreme brevity, and our limited influence over how it unfolds, is the only one we'll get a shot at. In the process of going further in his analysis, i.e. of growing to greater independence and productivity,his neurotic symptoms will cure themselves."
